Hospital at Home
What is it?
Bromley Hospital at Home is a collaborative, integrated model of care that delivers a range of monitoring and interventions in the community that would otherwise be delivered in an acute hospital.
The service model is designed to meet the needs of the local population and support Bromley patients by offering patients hospital care at home.
How do I access it?
Referrals are made by clinicians from One Bromley partners, with the aim to avoid acute hospital admission and early supported discharge.
